Dana-Farber Cancer Institute The Oncology Nurse Navigator (ONN) uses oncology-specific clinical knowledge to offer individualized guidance to patients, families and caregivers to foster independence during the cancer journey. Using the nursing process, an ONN assesses patient needs and provides education and resources to facilitate appropriate and timely quality health and psychosocial care throughout all phases of the cancer continuum. (ONS definition). The ONN creates a patient-centered and compassionate environment that inspires and motivates. The DFCI Cancer Hematology service provides clinical support for patients with cancer who have diseases of the blood, including non-malignant disorders of bleeding, thrombosis, platelets, red cells and white cells, as well as hematological disorders that emerge or complicate cancer treatment. We work with primary providers to optimize clinical outcomes and long-term outcomes. Located in Boston and the surrounding communities, Dana-Farber Cancer Institute is a leader in cancer research and patient care. Participates in day-to-day operations of the program including patient education and call triage Participates in program evaluation and development of strategic goals consistent with DFCI’s mission and professional values Participates in QA/QI projects for the program Supports the research activities of the program Delivers expert clinical care through evidence-based practice partnerships with patients, families and colleagues Assumes nursing care responsibilities for a defined group of patients within the program, including assessment, education, coordination, and documentation Facilitates individualized care considering functional status, cultural factors, health literacy, psychosocial, reproductive/fertility, and spiritual needs Supports smooth transitions from active treatment into survivorship, chronic cancer management, or end-of-life care Assists patients with cancer on treatment goals, advanced directives, palliative care, and end-of-life concerns using an ethical, nonjudgmental framework Acts as a liaison between patients, families, caregivers and providers to optimize outcomes Advocates for patient-centered care including shared decision making and goals of care Provides psychosocial support and facilitates appropriate referrals Contributes to ONN program development, implementation, and evaluation Tracks and monitors metrics and outcomes in partnership with administration Functions as the primary nursing contact in the physician practice setting Engages in collaborative decision making and data-informed practice Manages patient care across the continuum, including new patient calls and referrals Works collaboratively as a member of the health care team Monitors environment of care for patient safety and regulatory compliance Maintains evidence-based clinical practice aligned with nursing policies and standards Provides care based on age-specific needs Follows the Code of Ethics for Nurses and complies with licensure/registration requirements Fosters an environment sensitive to diverse populations Professional Development

Identifies areas for professional growth Formulates professional goals and methods to achieve them Represents the Nursing and Institute committees, including the Program Nurse Committee Supports research activities within Nursing and Patient Care Services and the Institute Meets annual evaluation requirements and competencies Collaborates on professional reports, articles, and presentations Pursues active membership in local and national professional organizations Committed to continuous learning and improvement through inquiry, innovation and mentoring Licensed as a Registered Nurse in the Commonwealth of Massachusetts Baccalaureate degree required; nursing baccalaureate preferred Minimum of 1 year (or equivalent) oncology nursing experience Certification in Oncology Nursing (OCN, AOCN, CPON) preferred Knowledge, Skills, And Abilities Required

The hiring range is based on market pay structures, with base salaries determined by factors such as business needs, market conditions, internal equity, and the candidate’s experience and qualifications. For union positions, the pay range is determined by the Collective Bargaining Agreement (CBA). The stated range: $42.65/hr - $105.81/hr.
