Monogram Health is a next-generation, value-based chronic condition provider serving patients with chronic kidney disease and end-stage renal disease. The Clinical Performance Lead will support clinicians and front-line staff in caring for our members and contribute to continual clinical performance improvement.

Responsibilities

Clinical Interventions – 30%

Review, update, and manage clinical interventions (CI), ad hoc tasking, and messages concerning visits. Document CIs, coach and assist RNs, develop action plans with RNs, outreach to providers, obtain updated lab work, and coordinate with team to complete tasks.

Assist APPs with requests and provide Salesforce coaching/education on charting or RN visit tasks.

Review records from sources (Athena, Health Gorilla) for high-risk/post-discharge members to inform RNs at visits; review labs and notes for care management review.

Perform concurrent review of hospital calls, conduct root cause analyses on certain members, and attend leadership meetings as needed (e.g., MPE).

Delegate actions to the Patient Support Specialist team for obtaining office visit records.

Patient Care and Treatment – 15%

Review and approve care plans; discuss patient care with field clinicians and address clinical escalations.

Facilitate case rounds and high-utilizer reviews; document follow-up tasks and coordinate scheduling for high-risk members.

Perform data analysis related to records and labs; address charting and user requests; document member conversations as needed.

Delegate actions to the PSS team for prescriptions and labs as appropriate.

Quality Assurance and Compliance – 30%

Delegate to PSS for lab result requests; audit charts and ensure complete documentation.

Analyze data from reports and create data spreadsheets; perform daily documentation reviews and provide guidance on IEs & PHDV.

Review and correct care plans to ensure SLA compliance; provide care coordination support for escalations and patient outreach.

Training and Communication – 25%

Provide guidance on processes, workflows, and documentation to field staff; communicate organizational changes.

Attend and prepare for routine operational and clinical meetings; create slides/content for meetings.

Support scheduling, communications, and reporting needs; build reports for specific member needs.

Position Requirements

Current, active unrestricted license as a Registered Nurse in assigned states/territories (compact/multi-state licensing may be required).

Minimum 2 years of RN experience within the last 5 years, or equivalent clinical experience.

Demonstrated knowledge and proficiency using technology.
