Ensure completion of patient databases, treatment plans, progress notes, discharge summaries, and other essential documentation in accordance with federal and state standards and agency policies. Provide patients with orientation to program objectives, phase requirements, rules, rights, and services. Assess and place patients according to treatment phases per standards and policies. Keep clinical documentation and charts updated per established procedures. Counsel patients in crisis, identify resources, and support coping. Counsel patients who may not comply with program requirements to increase awareness of consequences. Arrange referrals to community resources when appropriate. Perform treatment plan reviews and participate in case conferences. Develop knowledge of confidentiality, methadone treatment, and counseling licensure requirements. Submit internal and external correspondence for supervisor review. Complete intakes, facilitate group counseling, and conduct urine drug screens with proper chain of custody. Collect necessary documentation to maintain policy compliance. Submit monthly and quarterly statistical reports. Attend supervision, team meetings, and complete required trainings. Other duties as assigned. Qualifications
Counselor I: GED/High School diploma or higher; CDCA or higher certification in Ohio; licensure options as applicable; competency plan if less than one year of experience Counselor II: Associate degree or higher; LCDC II/III, LMFT, LPC, LSW (depending on licensure); Ohio licensure as applicable Counselor III: Master’s degree or higher; LICDC, LISW, LPCC (independently licensed) in OH Education: Required: High School/GED or better.
