We are seeking a full-time, experienced and compassionate licensed Clinical Social Worker or Mental Health Counselor dedicated to supporting high-risk behavioral health patients aged 18-64 who have had a psychiatric inpatient stay or Emergency Room visit as they step down to a community level of care. Job Summary
The primary responsibility of this role is to conduct post-discharge follow-up calls and support patients as they transition to community behavioral health services, providing a targeted, short-term care management intervention. The Clinician will support Mass Health Medicaid quality metrics for follow-up care after a Mental Health Inpatient or Emergency Department discharge. This role offers a meaningful opportunity to impact patient care and drive positive health outcomes. Qualifications
What You\'ll Bring
Required
Current Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) required. 1+ year of care management experience in a Behavioral Health setting required. Ability to work autonomously in a remote environment while collaborating with a team. Preferred
1+ year of experience in counseling patients or coordinating behavioral health support services, with exposure to an SMI or SUD population preferred. Additional Job Details
Working Model and Location Details
40-hour per week schedule: Monday through Friday 8:30am-5:00pm with some flexibility. Primarily remote; routine on-site meetings every other Tuesday at Assembly Row, 399 Revolution Drive, Somerville, MA. Remote Type

$66,206.40 - $96,304.00/Annual Grade
