Overview

The Nurse Practitioner/Physician Assistant is accountable to facilitate a quality hospital experience and outcome for patients within a highly specialized team in Neurointerventional Radiology and Vascular Neurology. The APP will be responsible to the Medical Director of Patient Services and oversee the progress of a specific caseload of patients across the care continuum. The role includes acting as an educator, consultant, and liaison to healthcare providers caring for patients throughout the care continuum. The APP will care for ischemic and hemorrhagic stroke patients and other cerebrovascular disease, including care during the hyperacute phase. Work locations include the Emergency Department, Stroke Service, and Neurointerventional Radiology. Responsibilities

Manage a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care across settings.

Oversee care coordination in acute, home, and ambulatory care settings as feasible.

Provide support, education, and encouragement to help the healthcare team achieve mutually agreed healthcare goals.

Demonstrate advanced physical and psychosocial assessment skills and knowledge of family dynamics; provide direct care in acute care settings.

Perform advanced practice skills under the supervision of the Medical Director; provide expert clinical consultation and participate in education, clinical research, and mentoring of healthcare providers.

Facilitate patient and family education in areas of expertise.

Complete mandatory safety infection control, BLS, and ACLS/PALS training annually; maintain prescriptive privileges per licensure.

Emergency Department – Hyperacute patients: respond to stroke codes, participate in acute revascularization therapies, obtain NIHSS and Modified Rankin Scores certifications, coordinate hyperacute care with ED/Neurology/NIR, assist in pre-procedure evaluation and room preparation, scrub and participate in procedures, assist Neurology residents with admission H&P and orders as needed, assist in MRI processes when indicated.

Stroke Unit – care for ischemic and hemorrhagic stroke patients with daily rounds, exams, progress notes, and discharge planning; serve as a resource for Neurology residents.

NIR Service – assist with consent, pre/post-procedure H&P, discharge planning and instructions; become proficient in sterile technique, radiation safety, device handling, arterial/venous access, catheter/wire manipulation, and closure techniques as applicable.

Minimum Qualifications

Basic knowledge:

Master of Science degree, graduate of accredited Nurse Practitioner or Physician Assistant program. Specialty certification required for Nurse Practitioners. Baccalaureate in Nursing. BLS and ACLS certification.

Experience:

One to two years related APP experience in an acute care hospital setting (preferred).

Independent Action:

Performs independently within department policies and practices; refers complex problems to supervisor when clarification is needed.

None.

Pay Range:

$130,000.00–$178,132.50
