Summary The Nurse Practitioner is accountable to facilitate a quality hospital experience and outcome for patients within a cost‑effective time frame. The Nurse Practitioner will be responsible to the Medical Director of Patient Services and will oversee the progress of a specific caseload of patients across the care continuum, including pre‑admission and post‑discharge care when feasible. Acts as an educator, consultant, and liaison to healthcare providers caring for the patients across the care continuum.
Responsibilities
Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.
Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model.
Oversees care coordination in acute, home care, and ambulatory care settings as feasible.
Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education, and encouragement.
Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics.
Provides direct care to patients in acute care, ambulatory care and home care settings.
Performs advanced practice skills under the supervision of the Medical Director.
Provides expert consultation for clinical issues, promoting increased knowledge and skills for the consultee.
Participates in the education, clinical research, and mentoring of healthcare providers.
Facilitates patient and family education in area of expertise.
Completes mandatory safety infection control, BLS, and ACLS/PALS annually.
Maintains Prescriptive privileges per licensure.
Qualifications
Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or license as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
Bachelor’s degree in Nursing.
Master’s of Science Degree from an accredited Nurse Practitioner program.
Specialty certification required.
Certified in Basic Cardiac Life Support (BLS).
Two years related experience as RN in an acute care hospital setting; preferred 1–2 years experience as a Nurse Practitioner.
Work Environment and Physical Requirements General hospital environment with occasional stressful conditions associated with patient care.
