Summary The Nurse Practitioner is accountable for facilitating a quality hospital experience and positive outcomes for patients within a cost-effective timeframe. The NP reports to the Medical Director of Patient Services and oversees a specific caseload of patients across the care continuum, including pre-admission and post-discharge when feasible. The NP serves as an educator, consultant, and liaison to healthcare providers caring for patients across the continuum.
Responsibilities Demonstrates a fundamental grounding in nursing theory and practice with mastery of knowledge within a defined content area. Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings. Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model. Oversees care coordination in acute, home care, and ambulatory care settings as feasible. Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education and encouragement. Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics. Provides direct care to patients in acute care, ambulatory care and home care settings. Performs advanced practice skills under the supervision of the Medical Director. Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee. Participates in the education, clinical research and mentoring of healthcare providers. Facilitates patient/family education in area of expertise. Annually completes mandatory safety infection control, BLS, and ACLS/PALS. Prescriptive privileges per licensure.
Minimum Qualifications Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing. Master of Science Degree, Graduate of accredited Nurse Practitioner program. Specialty certification required. Baccalaureate in Nursing. Certified in Basic Cardiac Life Support (BLS). Demonstrates knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Experience Two years related experience as RN in acute care hospital setting and preferred 1-2 years experience as a nurse practitioner.
Work Environment and Physical Requirements General hospital environment with occasional stressful conditions associated with patient care.
