skilled Customer Success Representative
to join our team and advocate for our home health and hospice services. At Health Link Home Health and Hospice,
your primary responsibility will be to help leverage Health Link's extensive relationships in the community to build and reinforce trust in our home health and hospice programs. Home Health is about trust - for nurses, social workers, and doctors to trust us with their patients, we must keep our promises and live out our values. Your role is to assist the front line of our home health team and give more patients the opportunity to be cared for by our clinicians. You will collaborate with our Marketing Team and Director of Community Relations to execute sales and marketing plans and achieve our goals. Success in this role requires a strong grasp of sales and marketing, and familiarity with the home health and hospice industry is a plus. Experience is not mandatory; rapid learning and adaptability are essential. Health Link has been a trusted community partner in the Bay Area for nearly 20 years, yet we operate with a startup mindset. Focus, commitment, integrity, and hard work will lead to success. Responsibilities Implement and support a comprehensive marketing and sales plan for Health Link Home Health. Develop and maintain relationships with community partners, including hospital teams, doctor offices, clinics, senior living communities, and skilled nursing facilities. Participate in ride-alongs with team members to respect existing relationships and develop your own. Schedule meetings, conduct in-person visits, and follow up diligently. Coordinate with the intake team to manage referrals and serve as a liaison with community partners. Represent Health Link at community events to increase awareness and network. Manage personal budget, track expenses, and optimize resource use. Identify and develop new referral partnerships. Maintain constant communication via phone, respond promptly, and manage your schedule and CRM diligently.
