Nuvance Health . Summary: Provides professional care to patients utilizing the nursing process in collaboration with other healthcare team members. Works on a per diem basis. Responsibilities:
Provides assessment of physical, emotional, socio-economic, and environmental needs of patients and their families. Identifies problems utilizing patient database, nursing process, and physician's plan of treatment. Develops a care plan and explains diagnosis and treatment implications to patients and families. Observes, evaluates, and reports patient conditions to the physician, including reactions to treatments. Assesses health status through observation and interviews; counsels families on wellness principles and related fields. Performs screening and diagnostic procedures; assists physicians during examinations and treatments. Interprets test findings, makes referrals, and obtains laboratory specimens. Attends continuing education and participates in Quality Management programs. Fulfills all compliance responsibilities and performs other duties as assigned. Other Information: Required: Current RN License in Connecticut Minimum Experience: 0-6 months Desired: BSN and experience in a qualified home health agency. Working Conditions: Manual skills, occupational risks, exposure to dirt, odors, noise, and human waste.
