8:00 AM - 4:30 PM Responsibilities and Qualifications
Experience 12 months of role experience is required with some in the last 12 months. Must have at least 10 months of Home Health experience. Experience with Oasis is required from any number of months. Experience with Epic is preferred from any number of months. Requirements Candidates must have a license (required after submission). This role will require floating to multiple locations. Local & travel allowed. Candidates living
Flu vaccination required for submission. Religious and medical declinations accepted. Pre-employment modules may be required for this role. Additional Details All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility. References must cover at least 1 full year of relevant experience in specialty within the past 3 years. Shift & Scheduling: This facility cannot accommodate 4x10s or 3x12s. Credentials Cannot have any hits/suspensions on license or any background hits from any point in the past. Government-Issued Photo ID. BLS Must be AHA (Instructor, Provider, or RQI) or “Heart&Stroke” certification from Canadian Heart & Stroke Foundation ONLY. Additional Details – Unit Types of Patients on Unit: Dementia, Parkinsons, ALS, MS, Ortho post-op, Post-Stroke Technology Used on Unit: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/Oasis Unit/Facility Details: West Portland OR and Surrounding Areas. May visit up to 6 patients per day; 30 productivity points standard. Previous Employment Requirements Cannot be currently or recently (within 6 months) employed with network - Must have left in good status, if former employee. Special Requests RTO must be listed at time of submission. Traveler is expected to have their own transportation. System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.
