Shift:

8:00 AM - 4:30 PM

Experience

12 months of role experience is required with some in the last 12 months. 10 months of Home Health experience is required with some in the last 24 months. Travel experience is required from any number of months. Experience with Oasis is preferred from any number of months. Experience with Epic is required from any number of months.

Requirements

Candidates must have a license (required for submission). This role will require floating to multiple locations Local & travel allowed. Candidates living

Flu vaccination required after submission. Religious and medical declinations accepted.

Additional Details

RTO must be listed at time of submission.

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Cannot have any hits/suspensions on license or any background hits from any point in the past.

Home Health PT - Napa CA and Surrounding areas

This facility cannot accommodate 4x10s or 3x12s.

Weekend Required: 1 Weekend Shift Per Contract

On Call not required.

Block Schedule: No

Years of Experience Required: 1 year of skilled PT experience.

Clinician to Patient Ratio - 1:1 (*May visit up to 6 patients per day; 30 productivity points standard)

BLS: Must be AHA (Instructor, Provider, or RQI) or “Heart&Stroke” certification from Canadian Heart & Stroke Foundation only.

Special Requirements

Traveler is expected to have their own transportation. System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50+ miles. Oasis is preferred, but not required; please note if candidate does not have Oasis experience

Additional Submission Requirements

Minimum of 2 supervisory references covering 1 year (6 months for CNA’s) of employment total within the last three years within a comparable specialty/setting that they are booked

All travelers are expected to float between all facilities within a given state. travelers’ refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours.

5 days maximum time off

2 references from last 3 years (Manager/Supervisor reference type) - required for submission
