Licensed Social Worker (LSW), Ryan White HIV Medical Case Management Program - Sign-on Bonus $1200

Job Summary:
Provides direct service to clients enrolled in the Early Intervention Program. Essential Job Duties & Responsibilities:
Provides case management assistance, advocates for client caseloads, and provides limited supportive counseling services. Responsible for case plan development and case conferences. Maintains case records on clients and reports service statistics. Collaborates with clinical staff to coordinate treatment and case planning. Transports patients to and from appointments, as well as Linkage to Care. Conducts intakes for the Early Intervention Program. Attends community fairs and collaborates with other agencies to gather information about referral sources and distribute information about Circle Health Services. Promotes effective communication within and between departments through teamwork. Attends departmental and Circle Health all staff meetings, contributing to decision-making through active participation. Assists with scheduling medical appointments for HIV patients and makes reminder calls. Works with Ryan White, attending monthly meetings and serving on at least one committee. Collaborates with medical and dental staff to meet Ryan White standards of care. Coordinates schedule and support for HIV volunteer medical providers. Prepares patient records, labs, and referrals for volunteer providers. Facilitates lab tests as scheduled and medication pick-up, including scheduling with CVS/ProCare pharmacy. Networks with all Ryan White providers and service representatives. Completes applications for OHDAP and other patient assistance. Ensures patient files comply with Ryan White Part A guidelines and Circle Health Services protocols. Provides necessary documentation in patient charts and is responsible for preparing new patients, audits, and quarterly quality assurance reviews. Other Job Duties & Responsibilities:

Valid driver license and auto insurance coverage with limits of $1,000,000/$300,000 bodily injury. Bachelor’s degree in Social Work with an LSW or a Master’s degree in Social Work with an LSW. Clinical license in good standing with the state of Ohio. Two or more years of related work experience. Willingness to work with HIV+ population. Core Competencies:

This job operates in a professional office environment and routinely uses standard office equipment. The employee is regularly required to speak and hear, frequently required to sit for extended periods, stand, walk, use hands and fingers, and reach with hands and arms. It requires the ability to lift files, open filing cabinets, and bend or stand on a stool as necessary. Travel and work outside of normal hours may be required for meetings, presentations, trainings, and other events. Supervisory:
