Overview

Intensive Behavioral Health Medical Case Manager, Ryan White Program — full-time position at Signature Health. The role provides client-centered services that link clients with medical, psychosocial, and other needed services under the supervision of the Ryan White Program Manager. The goal is to address barriers to consistent medical care and medication management to support viral load suppression. Responsibilities

Provide comprehensive, strengths-based, client-centered assessment of behavioral health needs. Develop an individualized care plan specific to behavioral health needs to improve medical outcomes. Coordinate medical and behavioral health services required to implement the plan. Evaluate crisis situations and apply appropriate interventions. Identify at-risk clients and develop plans to prevent psychological and/or physical health barriers. Educate clients, families, and staff about care, prevention, and treatment enhancement techniques. Continuously monitor clients to assess the efficacy of the plan; periodically re-evaluate and adapt plans as needed. Provide services that link clients with health care, psychosocial, and other services, including benefits/entitlement counseling and referrals to programs (e.g., Medicaid, Medicare Part D, State programs). Perform ongoing assessment of high-needs or at-risk clients and family members’ needs and support systems. Provide treatment adherence counseling to support readiness for and adherence to complex HIV treatments. Maintain client charts and ensure documentation meets contractual and Ryan White program requirements, including contact type, duration, and frequency of encounters. Advocate for clients and review utilization of services; maintain clear communication with the treatment team and referral sources. Uphold client rights and confidentiality and maintain high ethical standards. Participate in Quality Improvement and Utilization Review activities; maintain professional and ethical standards. Meet or exceed productivity and direct service expectations; comply with agency policies and procedures. Maintain HIPAA compliance and promote patient confidentiality and right to privacy. May perform telehealth services as determined by Signature Health; complete required training and productivity/documentation requirements. Participate in staff meetings and development programs; demonstrate reliable attendance and punctuality. Other duties as assigned. Requirements

Knowledge & Experience

Ohio license for LSW or LISW. Formal training and practical experience in medical case management models; core and specialty competencies working with HIV/AIDS clients, and with mental health and substance use disorders. Experience working with HIV+ and LGBTQ+ communities preferred. Knowledge, awareness, sensitivity, and competence regarding implicit bias, cultural humility, health disparities, LGBTQ+ communities, people of color, substance use disorders, sexual health, and trauma-informed care. Reliable transportation and willingness to travel throughout Cuyahoga County and attend meetings/trainings outside the county as needed. Valid Ohio driver’s license and good driving record; able to work in varied environments. Excellent oral and written communication skills; experience in behavioral health settings providing consultation, intervention, and referrals for psychological/psychiatric problems. Ability to make quick and accurate clinical assessments and to design and implement clinical pathways and protocols for chronic condition treatment. Working Conditions

Work is normally performed in an office/clinical environment; hours may require evenings and extended hours. Regularly requires sitting, standing, telephone and computer work; hearing and vision adequate to perform duties; dexterity to operate a PC and office equipment. Occasional lifting up to 15 pounds; possible exposure to bloodborne pathogens; frequent movement, bending, reaching, and computer use.
