Supports the mission of the department by ensuring that the right patients receive the right medication in a safe and timely manner. Clinically reviews medication orders to ensure safety and appropriateness; intervenes with the prescriber when needed; enters orders into the Pharmacy Information System; supervises pharmacy technicians and interns to meet the daily needs of patients; and provides drug information to medical staff and patients as requested. Essential Duties & Responsibilities

Clinically reviews all medication orders to ensure medications entered into patient profiles are safe for administration. Assesses appropriate drug selection, allergies, dosage, route, diagnosis, relevant laboratory results, therapeutic duplication, drug interactions, and patient age. Intervenes with appropriate health care professionals when necessary; documents clinical interventions per departmental policy. Enters reviewed orders into the Pharmacy Information System with attention to administration by the nursing staff. Supervises pharmacy technicians and interns to ensure daily patient needs are met; educates staff on safe medication practices. Provides drug information to medical staff and patients as requested or when needed. Manages day-to-day drug distribution, including controlled substances, IV medications, extemporaneously compounded medications, repackaged medications, and investigational drugs; uses relevant technology to perform duties. Serves as a liaison between the pharmacy department and health care providers. Maintains professional development, completes continuing education requirements, and participates in performance improvement activities. Performs assigned projects efficiently and demonstrates competency across practice areas. Incorporates Lahey Clinic Guiding Principles, Mission, and Goals into daily activities; complies with policies and behavioral expectations; maintains courteous interactions with colleagues and patients. Contributes to department and interdepartmental quality improvement; completes mandatory education; performs other duties as needed. Minimum Qualifications

Education:

Must be a graduate of an accredited college of pharmacy with a current MA registration in Pharmacy. L icensure, Certification, Registration:

Must possess a valid MA pharmacist license. Skills, Knowledge & Abilities:

Demonstrates the ability to be thoroughly trained to meet job requirements. Experience:

Hospital pharmacy experience is preferred; working knowledge of basic health system pharmacy concepts, including drug therapeutics.
