Care Manager, RN - ECM, Palm Desert (Remote)
Overview: Join IEHP as a Care Manager, RN - ECM in Palm Desert (remote). Under the supervision of ECM Department Leadership, this position provides high-quality, effective care management to IEHP members. Responsibilities include outreach to Members, engaging Members in therapeutic interactions to promote healthy behaviors, coordinating care, linking to resources, collaborating with community professionals and organizations, ensuring quality of care, seamless transitions, and facilitating the right care at the right time. This role works with ECM team, Members, families, other professionals, and the designated health care organization (HCO) medical team. The position models whole health principles of relationship-based care and promotes education and understanding of physical health and healthy behaviors within IEHP and the community. Responsibilities
Provide high quality, effective care management to IEHP members in collaboration with the ECM team Engage with Members through outreach and skilled therapeutic interactions to promote health behaviors Coordinate care and link to community resources; work with professionals and organizations to ensure quality of care Ensure seamless transitions of care and facilitate the right care at the right time Collaborate with Members, families, ECM team, and the designated HCO medical team Model whole health principles of relationship-based care and promote education about physical health and healthy behaviors Education & Requirements
Three (3) or more years of care management experience in a health care delivery setting required Experience in a Health Care Organization or Managed Care setting preferred Minimum of one (1) year clinical experience in an acute care facility, skilled nursing facility, home health or clinic setting preferred Associate’s degree in Nursing from an accredited institution required Bachelor’s degree in Nursing from an accredited institution preferred Active, unrestricted RN license issued by the California BRN required Key Qualifications
Valid California Driver’s license and automobile insurance; must maintain driving record to drive company vehicles per IEHP standards Knowledgeable in evidenced-based communication such as Motivational Interviewing or similar empathy-based strategies Understanding of and sensitivity to multi-cultural communities Knowledge of self-management philosophies related to chronic conditions Understanding of bias and commitment to addressing it in health care Bilingual (English/Spanish) preferred; strong interpersonal skills Excellent interpersonal communication; conflict resolution abilities Ability to engage Members and Providers over the phone; capable of functioning as part of an autonomous team Ability to perform minimal physical activity as needed Position eligible for telecommuting/remote work upon HR approval; may be required to report to IEHP main campus for in-person meetings Benefits
