Overview

We are seeking a dynamic physical therapist to join our flexible workforce team (FWF) and provide orthopedic PT services to adolescents through adult clientele throughout the metro area. The FWF rehab team provides staffing coverage across multiple outpatient orthopedic therapy clinics in the greater Twin Cities metro area. Preferred candidate will have Mechanical Diagnosis and Therapy (MDT) training. Where MDT training and experience is limited, Fairview offers mentoring and development opportunities for professional growth. Responsibilities

The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation Gathers pertinent data Performs examination Determines treatment diagnosis Develops an individualized plan of care through collaboration with the patient and family Analyzes data to identify needs and potential risks Determines prognosis for recovery Establishes individualized goals with patient and/or caregiver Individualizes plan of care considering developmental level and cultural differences Implements and modifies plan of care based on reassessment and patient response Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care Establishes appropriate discharge plan Partners with patients and families to provide skilled intervention Implements plan of care Alters treatment/plan of care to reflect change in patient status and response to treatment Assesses patient and/or caregiver learning needs and provides appropriate education Qualifications

Required

Physical Therapist in the state of Minnesota or temporary license in the state of Minnesota Basic Life Support (American Heart Association or Red Cross) Preferred

Doctorate of Physical Therapy Mechanical Diagnosis and Therapy (MDT) training and experience 1 year experience Job Details

FTE 0.8, authorized to work 64 hours per pay period Coverage at various locations across the greater Twin Cities metro area including Apple Valley, Blaine, Bloomington, Burnsville, Eagan, Edina, Eden Prairie, Fridley, Golden Valley, Hugo, Maple Grove, Maplewood, Minneapolis, St. Anthony, and Woodbury Benefits

The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. The organization values pay equity and considers internal equity when making offers. Hiring at the maximum of the range is not typical.
