The RN Case Manager for Home Health provides nursing care in accordance with the client's plan of care, and includes comprehensive health and psycho-social evaluation, monitoring of client's condition, health promotion and prevention, coordination of services, teaching and training activities and provision of direct nursing care and other duties as deemed necessary. Education: Graduate of accredited nursing program Experience: 2 years of RN experience with one (1) year experience with adult IV expertise required Licenses and Certifications: Registered Nurse (RN) by Board of Nurse Examiners for Texas or compact state; Valid Texas driver's license and proof of insurance; Basic Life Support (BLS) Preferred
Education: BSN Experience: Other RN experience including home health; knowledge of InterQual and TMF care criteria Job Duties
Provide direct nursing care, including patient observation and delivery of prescribed treatments Conduct initial and ongoing comprehensive assessments of home health patient needs, including OASIS assessments at appropriate time points Evaluate outcomes of patient care and identify discharge planning needs; consult with other disciplines; adjust nursing care processes as necessary Maintain patient records and charts to ensure accurate records of treatment Provide instructions to patients/families for use of products and services Prepare, administer, monitor, and discontinue IV fluids; manage IV delivery systems Administer medications and treatments as prescribed Perform phlebotomy, collect laboratory specimens, and conduct point-of-care testing Ensure delivery of quality patient care that meets standards and regulatory guidelines Maintain a safe, comfortable, and therapeutic environment Participate in Continuous Quality Improvement (CQI) programs Provide supervisory visits for Home Health Aides and LVNs as required Participate in on-call rotation and on-call emergencies as per policy Perform other duties as assigned Security and EEO Statement
