Primary Care Provider, Nurse Practitioner / Physician Assistant - Full-Time | PACE Program at WelbeHealth. As part of an interdisciplinary, mission-driven team, you can build deeper, long-term relationships with fewer patients and see the impact of your care every day. Responsibilities
Provide comprehensive primary care within the PACE program, delivering holistic, patient-centered care to eligible seniors. Establish and nurture long-term relationships with a smaller patient panel to support ongoing health needs and outcomes. Collaborate with an interdisciplinary team to coordinate care and manage complex cases, including home visits as part of regular hours. Participate in rotating on-call coverage (a few nights or weekends per month) available by phone. Qualifications
Licensed Nurse Practitioner or Physician Assistant with at least two years of experience working with the elderly. Prescriptive authority, DEA registration, and eligibility to serve Medicare/Medi-Cal participants. Compensation and Benefits
Competitive compensation of $145,399 - $174,478 with quarterly bonus. Generous time off: weekends off, 17 days PTO, 6 sick days, 12 paid holidays. CME reimbursement; 401K savings with match, plus equity grants. Medical insurance coverage (Medical, Dental, Vision). Regular hours: 8am-5pm, Monday through Friday, including home visits.
