Primary Care Provider, Nurse Practitioner / Physician Assistant - Full-Time | PACE Program. Responsibilities
Deliver patient-centered primary care within the PACE program, emphasizing comprehensive, holistic care.
Develop ongoing, relationships-driven care for a smaller panel of patients, enabling visible impact of services.
Provide care through a value-based model, including in-clinic and home visits as scheduled (regular hours are 8am-5pm, Monday through Friday).
Participate in an interdisciplinary team approach to care planning and execution.
Manage rotating on-call responsibilities (a few nights or weekends per month, available by phone only).
Qualifications
Licensed Nurse Practitioner or Physician Assistant with at least two years of experience working with the elderly.
Prescriptive authority and DEA registration.
Eligible to serve Medicare/Medi-Cal participants.
Compensation and Benefits
Competitive compensation of $181,000 - $254,000 with quarterly bonus.
Generous time off: weekends off, 17 days PTO, 6 sick days, 12 paid holidays.
CME reimbursement and 401K savings with match, plus equity grants.
Medical insurance coverage (Medical, Dental, Vision).
Regular hours of 8am-5pm, Monday through Friday, including home visits.
On-call schedule rotating — a few nights or weekends per month, via phone.
