Overview

Pharmacy Clinical Specialist - Hematology/Oncology exp required - Houston Methodist West Hospital. This position is responsible for performing advanced clinical pharmacy duties enabling the delivery of optimal pharmacotherapy and ensuring safe, efficacious, and evidence-based use of medications for the populations served. The role involves daily reactive and proactive medication regimen reviews within designated areas of responsibility, supporting pharmacy managed protocols, formulary processes, and medication reconciliation. The specialist also precepts pharmacy students and residents and contributes to the organization’s mission to provide innovative, personalized, cost-effective pharmaceutical care in a culture dedicated to quality and safety. Responsibilities

Promotes a positive work environment and supports a dynamic, team-focused unit. Collaborates with other health care professionals to resolve problems and achieve patient-specific goals. Serves as a rotation preceptor for students and residents and mentors staff pharmacists to support clinical skill advancement. Processes orders accurately and efficiently, verifies drug appropriateness based on diagnosis, allergies, height, and weight; documents patient care activities and outcomes related to pharmaceutical care and clinical interventions. Provides patient-specific and general drug information support regularly. Serves as a member of Medical Staff Committees and contributes to committee goals. Quality/Safety

Collaborates with health care professionals to optimize drug therapy, minimize side effects, adverse reactions, toxicities, drug interactions, and incompatibilities using established guidelines. Delivers pharmaceutical care across the designated practice areas, including proactive reviews, patient counseling, transitions of care, medication acquisition support, and management of pharmacist-based dosing protocols. Participates in quality management initiatives (e.g., medication use evaluations, adverse drug reaction reporting, clinical interventions, medication center reviews, inventory documentation). Finance

Contributes to pharmacoeconomically sound medication therapy by upholding formulary standards and avoiding complications. Manages time effectively and prioritizes tasks within the specialty area. Growth/Innovation

Presents in-services for pharmacy, medical, nursing, and other staff; develops educational materials and programs for pharmacy staff and hospital personnel. Seeks opportunities for continual development as a Pharmacy Clinical Specialist. Contributes to design, analysis, and reporting of quality improvement findings; participates in research projects to advance quality of care and the pharmacy profession. Submits research work for publication, presentations, or posters. This job description is not all-inclusive; other reasonably related duties may be assigned. Graduate of an education program approved by the credentialing body for the required credential(s) indicated in the Certifications, Licenses and Registrations section. Work Experience

Must have one of the following: 1) Successful completion of an ASHP-accredited PGY1 residency or equivalent experience (three years of pharmacy experience per residency year); ASHP-accredited PGY2 or fellowship preferred; 2) Three years of clinical practice experience; 3) Meets HM standards of an ASHP-Qualified Residency Preceptor within one year of employment. Licenses and Certifications

Required: Pharmacist license for Texas (or eligible within 90 days) AND Pharmacist Preceptor Certification (or eligible within 90 days) AND Basic Life Support (BLS) certification (or Instructor) within 90 days. Licenses and Certifications - Preferred

ACLS or Instructor, PALS or Instructor, IV Certification, or Board of Pharmacy Specialty certification. Knowledge, Skills, and Abilities

Demonstrates competence in safe drug selection, evaluation, utilization, and distribution across patient groups (neonate to geriatric). Strong communication and customer service orientation; effective collaboration with patients, physicians, family members, and co-workers. Leads and coordinates activities within a major pharmacy service area and applies current evidence-based guidelines and innovative clinical practices. Experience with direct patient care activities and collaboration with nursing and medical staff; capable of performing research related to pharmacy practice and maintains credentialing with HM Academic Institute as applicable.
