Perform bio-psycho-social assessments, care coordination, and clinical interventions. Contribute to program development and discharge planning for patients. Effectively intervene with medically and psychosocially complex patients. Uphold Cottage Health Patient First Philosophy and align with the organization’s Mission and Vision. Qualifications
Education: Master’s in Social Work (MSW) from a CSWE-accredited School of Social Work (minimum). Licenses/Certifications: Current California Clinical Social Work License (LCSW) required upon hire. CCS paneling required within six months of hire. Preferred: CCS paneling and Certified Social Worker in Health Care (C-SWHC). Experience: Minimum 1-2 years of relevant clinical social work experience; 3-4 years preferred in a healthcare setting. About Us
