Care Partners are responsible for meeting the basic needs of patients by providing skilled levels of bedside patient care under the direction of the Registered Nurse. Care Partners also perform clerical duties necessary to the functioning of the nursing unit. Responsibilities and Requirements

General Patient Care

Carries out the patient’s plan of care as it relates to the patient’s physical needs under the direction of the Registered Nurse. Delivers care requiring broad knowledge of aseptic technique, medical terminology and equipment. Assists nurses and physicians with procedures. Maintains knowledge of and operates equipment used in patient care safely. Retrieves patient care items not routinely available on the unit; follows up on patient care needs. Performs completed unit clerical duties including coordination of designated unit activities, transcription of orders, medical record maintenance and serving as a unit receptionist. Prepares rooms prior to admissions and following discharges; completes patient belongings list on admission and transfer. Directs prompt and courteous attention to the needs of patients, families, and visitors. Rounds with purpose and engages in service recovery activities referring unusual problems to Nurse Manager/Designate. Rounds after dietary trays are passed to ensure meals are appropriate; takes appropriate action regarding late or wrong trays. Feeds and sets up trays as needed; maintains supplies and equipment; notifies appropriate departments to maintain a clean safe environment. Functions as a resource person to co-workers, patients/families, medical staff, and ancillary personnel.

Safety, Monitoring and Care

Correctly places restraints; observes for signs of constriction, changes to skin integrity, adequate breathing; provides ROM and skin care, toileting, and opportunities for socialization. Operates Cardiac Monitor: on/off, alarms/stand-by, obtaining strip, lead placement. Completes Point of Care Testing as approved; obtains vital signs including temperature; measures abdominal girth, weight, and intake/output. Mobility and Exercise: applies Pneumatic Air Stockings/Ted Hose; ambulates, turns and positions patients. Respiratory Care: assists with incentive spirometry and cough/deep breath; sets up oxygen therapy per RN direction; monitors pulse oximetry; completes tracheostomy care and suctioning; oral and tracheal care when stable. Wound Care: places patient on specialty bed; performs dressing changes; cares for pin care site; monitors traction weights. Bowel and Bladder Elimination: administers enemas; performs straight catheterization and catheter care. Hygiene: provides daily hygiene and grooming; mouth care; offers fresh water; performs perineal care and toileting.

Specimen Collection and Clerical

Obtains midstream urine, timed urine, sterile urine from indwelling catheter, urine specific gravity (5 South and 5 ICU), urine pH, sputum, blood, stool specimens. Clerical: answers phone promptly; directs physicians and hospital personnel; notifies new patient arrival; handles admissions/transfers/discharges documentation; maintains census and daily worksheet; communicates unit activities; ensures correct specimen labels and requisitions; transcribes orders and enters into computer; coordinates test/test orders with other departments.

Service and Professionalism

Demonstrates care and courtesy; provides prompt, respectful service; meets physical, spiritual, psychological and emotional needs of patients and families as directed by RN; responds to call lights promptly; communicates effectively and maintains confidentiality; participates in hand-offs and ensures quality care; supports unit workflow and cost-effective service; demonstrates personal accountability.

Safety/Infection Control and Compliance

Practices standard precautions; maintains a safe, clean, and therapeutic environment; adheres to dress code; reports risk management concerns; completes mandatory trainings and unit competencies; performs duties in accordance with hospital standards and policies; adheres to patient privacy and confidentiality.

Practice Agreement and Standards for Nursing Staff are posted for reference and understanding of expectations related to infection control, nursing practice, environment, and collaboration. The agreement emphasizes patient care quality, safety, and professional conduct. Minimum Qualifications and Requirements

MINIMUM EDUCATION: High School graduate or GED; at least 18 years of age. Post-high school vocational/specialized training preferred. MINIMUM EXPERIENCE: None REQUIRED COURSE: Completion of learning program with 80% passing score on written and skills performance examination. WORKING CONDITIONS and PHYSICAL REQUIREMENTS are described, including exposure to blood-borne pathogens, irregular hours, and the ability to lift 100+ pounds in certain tasks.
