Speech Therapist (ST) Home Health – Alameda County. We are seeking a compassionate, organized, and team-oriented Speech Therapist to join our Home Health team as a Full-Time or Part-Time (Per Diem). You will work with a dedicated team of healthcare professionals to make a meaningful difference in the lives of our patients. Responsibilities
Provide, coordinate, and oversee skilled speech therapy services as per the patient's individualized plan of care, in collaboration with the Interdisciplinary Team. Conduct initial and ongoing evaluations of patient status. Develop and implement treatment plans, monitor patient progress, and report changes to the Physician, Case Manager, and/or Director of Nursing. Deliver educational information to patients and their families regarding speech and language disorders and treatment plans. Maintain accurate clinical records and submit documentation in a timely manner per company policy. Adhere to all company policies and procedures to ensure the highest standards of care. Requirements
Current OT License to practice in California – Required Experience in Home Health or Hospice care – 1 Year Required Current CA Driver’s License Able to travel within a 10 - 15 mile radius on a regular basis Benefits
