Overview

Full-Time Hospice RN Case Manager position located in Morris County, NJ. Ennoble Care provides mobile primary, palliative, and hospice care with patients across multiple states. The RN Case Manager works as part of the interdisciplinary team to provide skilled nursing assessments, planning, and care to maximize comfort and health for patients and their families. Responsibilities

Manage a patient caseload assigned by the Patient Care Coordinator or designee Assume primary responsibility for the nursing process (assessment, planning, implementation, evaluation) for each patient in caseload Initiate and coordinate communication with attending physicians, Hospice staff, and other agencies to optimize care and resources for patients and caregivers Maintain regular communication with the attending physician regarding patient status Collaborate with team members to enhance the patient plan of care Document accurately and timely in the electronic medical records system Participate in Interdisciplinary Group meetings and contribute relevant information Coordinate physical care by teaching primary caregivers, volunteers, and employed caregivers, or provide direct care as appropriate Review caseload with Hospice nursing staff to address issues, support professional growth, and share feedback Attend patient care conferences and facilitate discussion of caseload issues Supervise and document NAC as assigned Inform the Patient Care Coordinator of unusual or potentially problematic issues Act as associate nurse during on-call periods or when covering for other primary nurses as needed Provide support at the time of death and during bereavement Participate in orientation and in-service training; engage in agency and community programs to promote professional growth Maintain familiarity with agency policies and state/federal hospice regulations Accept other assignments as appropriate Qualifications
