What We’re Looking For
Rock Creek Foundation seeks a highly motivated, independent, and relationship-driven Full Time Bilingual Case Manager to join our team. The Case Manager is responsible for assessing, planning, implementing, monitoring and evaluating actions required to meet the individual’s health and human services needed.
What You’ll Do
Assesses clients' social, emotional, financial, health, vocational, or other problems to develop rehabilitation treatment plan, or Individualized Plan goals for Person-Centered Plan Responsible for scheduling, attending, writing, and implementing Person-Centered Plan Complete Intake/Assessment process for all new DDA Program, Employment Services, CDS, PSS and Residential individuals in both English and Spanish Ensures that all Person-Centered Plan goals are inputted in Electronic Medical Records and monitors tracking on all goals Monitor monthly goals, activities and track progress, ensures that goals are CARF compliant Liaison between individuals, families, and any other community service provider Document case records and other agency required documents Assists with reporting, documenting and provides appropriate response time for all internal/external incidents per DDA/MHA Policies on Reportable Incidents Identify and coordinate behavioral services when needed, makes referrals as needed Prepares and maintains written case records, reports, performs case follow-up and closing, and performs other administrative tasks as required Assists in conducting all quarterly chart audits Conduct weekly and/or bi-weekly service notes in iCM to ensure compliance with QIO/CQL standards Coordinate and implement general departmental projects Responsible for ensuring staffing schedules and ratios are in compliance per individual’s plan/treatment plan Respond to and resolve customer complaints Attends all mandatory meetings Oversees the Electronic Medical Record Is on-call for all departmental needs Participates in and successfully completes all required trainings Provides training to all new employees and/or contract staff on iCM shift notes, attendance, etc. Oversees Quality Assurance measures at all assigned licensed sites Adheres to all County, State DDA, MHA, and MBON policies and procedures Other duties as assigned
What We Need
High School Diploma or GED; OR Bachelor’s Degree in a human service or related field of study; OR Associates Degree and (1) year of experience providing support to adults with ID/DD and SPMI (2) years of case management and/or coordination of care; OR equivalent combination of education and experience.
