Job Description: Pre-Admissions Coordinator Full-Time, Nights Job Description: Facilitates patient access to the appropriate level of care, health professional(s), and setting(s) to provide care and services for patient's assessed needs and the organization's capacity to provide care. Coordinates the placement of all patient admissions (elective and emergent) and patient transfers in accordance with standards and guideline. The ACC Nurse ensures the patients are admitted to the most appropriate unit to the patients’ care needs. This placement is based on admission criteria and bed availability. The ACC Nurse fosters a collaborative work environment through the sharing of information and demonstration of respect for all individuals. Continually advocates for patients.
