Inova Fairfax Sickle Cell Team is looking for a dedicated Experienced Social Worker Case Manager 2 to join the Case Management Team. The role ensures timely patient management and progression of care while serving as a liaison between patients, families, and providers. The position includes building relationships with community organizations, facilitating the monthly support group, providing limited short-term counseling as needed, connecting patients with external mental health resources, linking patients to resources addressing social determinants of health (food, transportation, financial, etc.), and assisting with blood drives and the annual town hall. Collaborates with the interdisciplinary care team, patients and families in the assessment/coordination of discharge planning needs, post-discharge planning, and transition from hospital to discharge setting, with ongoing community care. Initiates referrals to clinics, home health, hospice, SNF, acute rehab, LTAC, TCM, medical equipment and supplies; refers cases to resolve barriers to care progression. Documents relevant discharge planning information in medical records according to department standards and care management plans. Assesses patients' biopsychosocial needs through review of information, direct contact with patients/families and interdisciplinary team members. Communicates status of care plans with patients, families, interdisciplinary team members and other parties; tracks progress toward treatment goals, identifies concerns, and helps with problem solving and conflict resolution when needed. Ensures options to support a successful transition and essential care plan elements are documented and communicated for continuity of care. Seeks consultation from appropriate disciplines and community services to support discharge and ongoing community care plans. Assesses psychosocial risk factors and intervenes with emotional, social and financial consequences of illness and disability. Acts as a resource, providing counseling and interventions related to treatment and end-of-life decisions; advocates for patient/family autonomy and access to needed healthcare services. Education:
Master\'s Degree in Social Work Experience:
Minimum of 2 years of experience in clinical care or clinical case management. Certification:
Basic Life Support (upon start); Accredited Case Manager, Certified Case Manager (through CCMC or NASW), Certified Clinical Transplant Social Worker, or Licensed Clinical Social Worker. Preferred Qualifications
Two (2) years of inpatient, oncology, hospice, and/or palliative care case management experience is strongly preferred. Previous experience with sickle cell patients is also preferred. About Us
