Overview
Inova Fairfax Sickle Cell Team is looking for an experienced Social Worker Case Manager 2 to join the Case Management Team. This is a full-time role, Monday–Friday 8:00am–4:30pm with no evenings and no weekends. The role will split time between the Inova Fairfax Campus and Inova Loudoun Campus. Responsibilities
Collaborates with the interdisciplinary care team, patients and families in the assessment/coordination of discharge planning needs, delivery of post-discharge planning needs/services, transition of patients from the hospital to the discharge setting as well as ongoing care in the community. Initiates referrals to clinics, home healthcare, hospice, SNF, acute rehab, LTAC, TCM, medical equipment and supplies as indicated. Refers cases/issues appropriately to resolve barriers to care progression. Documents relevant discharge planning information in medical records according to department standards and/or care management plans. Participates in the assessment of patients\' biopsychosocial needs through review of information, personal contact with patients/families and interdisciplinary care team members. Communicates routinely with patients, families, interdisciplinary care team members and other appropriate parties regarding status of care plans, progress toward treatment goals, concerns, problems, and conflict resolution when necessary. Ensures all options to support a successful transition and critical elements of care plans are documented and communicated to patients/families and healthcare team to ensure continuity of care. Seeks consultation from appropriate disciplines and/or community services to assist with discharge facilitation and ongoing community care plans. Assesses psychosocial risk factors of patients/families and intervenes regarding emotional, social and financial consequences of illness and/or disability. Serves as a resource for counseling and interventions related to treatment and end-of-life decisions; advocates for empowerment and independence in healthcare decisions and access to needed services. Minimum Requirements
Education: Master\'s Degree in Social Work Experience: Minimum of 2 years of experience in clinical care or clinical case management Certification: Basic Life Support (upon start); Accredited Case Manager, Certified Case Manager (through Commission for Case Manager Certification or National Association of Social Workers), Certified Clinical Transplant Social Worker or Licensed Clinical Social Worker Preferred Qualifications
Two years of inpatient, oncology, hospice, and/or palliative care case management experience strongly preferred. Prior experience working with sickle cell patients also preferred. Benefits
