Coverage Area:
Vallejo Area In partnership with other clinicians, you will provide skilled services to evaluate and address the needs of individuals in their homes so they can live safely and productively in their home environment. What you’ll do as a Home Health Physical Therapist Case Manager
Perform an evaluation in the home and develop a care plan, coordinating with Physical Therapy Assistants (PTAs) to follow the plan. Help patients with mobility, movement, and regaining independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure a high level of care. Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge of local community resources Understanding of how illness/injury and culture affect health care Knowledge of state and federal homecare regulatory guidelines (preferred) Experience in home health care (highly preferred) Experience with high-acuity patients (a plus) Experience working with an interdisciplinary team (highly desired) Case management fundamentals (highly desired) Self-directed, empathetic, critical thinker with outstanding interpersonal skills Leadership, organization, and ability to work independently Compensation : $120,000 - $135,000 per year Wage details: Physical Therapist, Level 1: 120,000 to 135,000 plus a 10,000 annual bonus, paid quarterly. Starting pay is commensurate with relevant experience. 21st Century offers bonus plans, comprehensive health benefits, and 401k with up to 4% match.
