RN-Case Manager Schedule:
Full Time (24 RVEs/week), with benefits; Part Time (18 RVE); Per Diem (12 visits/week) *Non-traditional schedules available, e.g., Thursday-Monday Locations:
San Geronimo, San Quentin, Sausalito, Tiburon, Stinson Beach, Bolinas, Mill Valley, Corte Madera, Larkspur, Ross, Kentfield, San Anselmo, San Rafael, Woodacre, Lagunitas, Nicasio, Novato, Fairfax, Forest Knolls, Greenbrae, Inverness, Marshall, Muir Beach, Point Reyes Station Paid Training:
Must be able to train for 5 days, Monday-Friday, 9am-5:30pm in San Ramon Position Summary:
The Home Health RN monitors patients' overall health and, in partnership with a primary care physician, develops individualized care plans. RNs function as case managers, working independently to deliver quality healthcare, including customized patient education and specialized services such as wound therapy, post-surgical care, and ostomy management. We provide comprehensive internal operational support to handle tasks like medication reconciliation, DME orders, MD signatures, coding, lab follow-up, etc., allowing you to focus on patient care and schedule flexibility. The home care setting offers meaningful time with patients in their own homes. Requirements: Minimum 1 year RN experience, preferably in acute, SNF, Home Health, or Hospice Active CA Registered Nursing License AA, BSN, or MSN degree Reliable transportation Current CPR or BLS certification Benefits Include: Medical, Dental, Vision, 401K, Life Insurance, Holiday Pay, and PTO Compensation based on full-time schedule Do you have a current California RN license?
