RC, Patient Placement Specialist

In collaboration with the Manager of Clinical Operations for Patient Placement, Nursing Unit Clinical Managers and the Directors of Patient Care Services, the RC is responsible for the proactive planning, implementation and coordination of daily patient placement and throughput activities to maximize available bed space and access. Works collaboratively with all patient care departments and ancillary departments to assure census goals are met throughout strategic placement of patients based on the Bed Management Plan and Patient Population Guidelines. Provides input and collaboration in determining needs flex or open or close units to volume demands.
Education
Associate's Degree in Nursing - Required
Experience
1-2 Years Bed Management-Patient Placement Throughout - Preferred
License & Certification
Professional License with State of Practice - Required
CCRN, UM or CCM Certification - Preferred
Basic Life Support (BLS) - Required
Core Job Functions
Assist in forming teams for process improvement. Proposes new and innovative approaches for dealing with patient flow. Conduct daily huddles to coordinate the admissions and transfer of patients.
Accountable for analyzing data and utilizing resources and historical data to forecast and plan for capacity. Facilitates communication and joint decision making. Manages discharges and tracks patient access.
Effectively translates hospital strategy and objectives to nursing units and ancillary departments. Serves as a resource for development and implementation of patient flow. Mentors and promotes an environment that fosters learning and competence.
Demonstrates knowledge and skills to provide care appropriate to any age-related needs of the patient being served. Coordinates patient care needs with team members and other appropriate departments. Coordinates/oversees patient placement within the system and monitors house availability and assesses patient needs for house placement.
Demonstrates knowledge and practice of The Joint Commission, OSHA and CMS guidelines.
Follows the established policies to make patient assignments. Coordinates the movement of patients to assist with bed availability. Prioritizes the cleaning needs of patient rooms.
