to join our new team in the Bedford/ Lynchburg area!

Your Role as an RN Case Manager:
Travel within a 40-mile service area radius of the Bedford/Lynchburg office location for patient visits with generous mileage reimbursement paid out bi-weekly.
Coordinate and oversee care for hospice patients, ensuring comfort and dignity
Develop individualized care plans and collaborate with an interdisciplinary team
Provide education and emotional support to patients and families
Advocate for patients’ needs and ensure high-quality, compassionate care
Provide direct patient care in homes and contracted facilities
What You Bring:
Reliable form of transportation
Hospice or home health experience preferred, but not required
Strong clinical and assessment skills
Compassion, empathy, and a heart for serving others
