About the Role We are hiring a
Physical Therapist
to join our
Ortho-Based Outpatient in Home Transition Program
in partnership with
Luminis Health . This innovative care model focuses on short-term in-home rehab—typically 2–3 visits—prior to transitioning patients to outpatient care. This role offers a unique alternative to traditional home health: No SOC (Start of Care) visits No case conferences or OASIS documentation Orthopedic caseload only Focus strictly on evaluation and follow-up visits You’ll see approximately
6–7 patients per day
with a goal of

Requirements for this position include the following: Graduate (current or pending) from an accredited physical therapy program Active or pending state physical therapist license Comfort working independently in the home setting Professional Development
