As a
Home Health Social Worker , you will help seniors access essential services, provide emotional support, and guide families through healthcare, financial, and social resource navigation. Your involvement ensures a holistic approach to elderly care, improving quality of life and peace of mind. Key Responsibilities:
Conduct psychosocial assessments to identify clients’ needs and challenges. Connect seniors and families with community resources, support groups, and financial assistance. Offer emotional support, counseling, and advocacy to help clients cope with life changes. Collaborate with healthcare professionals, caregivers, and external organizations. Maintain thorough documentation of client interactions and progress. Qualifications:
Master’s in Social Work (MSW) and valid California license (LCSW preferred). Experience with geriatric populations or home health services. Strong communication, empathy, and resourcefulness. What We Offer:
