Site Coordinators/Case Managers needed for full-time position in Philadelphia County.
This 35-hour per week, year-round position supports pregnant and parenting students in a high school setting. The Site Coordinator/Case Manager oversees recruitment, enrollment, and delivery of contracted services, including quarterly home visits, monthly service hours, bi-weekly case management, and individualized student plans. They track attendance, grades, and progress using various databases, assist with childcare referrals, and connect students to social services and community resources.
The role involves evaluating academic progress, addressing parenting and life skills, and collaborating with school staff, families, and service providers to help students meet graduation goals. Knowledge of city systems such as Family Court, DHS, CCIS, and WIC is essential.
Position Requirements
BSW or MSW and two years of experience in social services and academic support for high school students are required.
