Position Purpose:
This position serves as the navigator for the Systems of Care program. The Systems of Care program is a grant-funded program that seeks to develop a collaboration amongst area service providers, identify mental health service gaps in services and needs within the community, work with the collaboration to develop programming to fill those gaps, and to develop a method of communicating and referring between providers to give clients the wrap-around services that they require for recovery. Under the supervision of the System of Care Coordinator, this role is intended to work with those agencies to facilitate this collaboration, develop the system of referrals and communication, market this collaboration to other local providers to increase engagement amongst providers, work to engage and incorporate individuals with lived experience into the collaboration, and develop/maintain the database that will increase communication and referrals between providers.
Qualifications/Basic Job Requirements:
Bachelor’s Degree in a behavioral health related field or equivalent experience and training Demonstrated experience working in a behavioral health related field Ability to coordinate and lead meetings with both internal and external resources Excellent analytical and continuous improvement skills, with the ability to analyze data and report on program performance Excellent communication skills, both verbal and written Strong interpersonal and relationship-building skills Proficient in use of computers and Windows-based programs Belief in the mission and vision of Rosecrance Essential Responsibilities:
Work hours prescribed and any additional hours deemed necessary for the responsibilities assigned. Develop partnerships with youth-serving organizations in the assigned geographic area. Seek out existing networking meetings in the community to promote the System of Care mission. Maintain communications with individuals and agencies involved in the Systems of Care collaboration. Develop and host on-going meetings with outside agencies and individuals to present and market the Systems of Care framework. Responsible for coordination of the implementation of software systems for managing regional resources. Coordinate the development and implementation of operational policies for the use of the Systems of Care Software across area agencies. Seek out and develop strategies to improve communication about the Systems of Care resources among area social service agencies and internal Rosecrance departments. Present workshops, lectures and training to patients, families, and staff and community groups. Represent the department in PI committees and organizational initiatives as determined appropriate by leadership. Become familiar with all related state and county contracts, and complete monthly reports as required. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance. Establish relationships with community organizations serving populations at higher risk of mental health disorders (e.g., YMCAs, teen centers, juvenile justice, etc.) to increase access points. Foster partnerships with both traditional and non-traditional access points to broaden the coalition’s reach. Coordinate the education and recruitment efforts to train community organizations on performing mental health screenings and integrating these practices into daily operations. Train clinical and non-clinical partners to utilize the IRIS system for referrals, ensuring proper follow-up services are provided to youth. Conduct ongoing analysis and feedback collection from youth clients, their families, providers, and coalition members to assess the impact and improve the model. Generate philanthropic support for sustainability and enhanced program needs, as needed Assume other related responsibilities as assigned by leadership. Schedule:
