Clinical Assessment RN - Homecare (Boston South) role at Amergis Healthcare Staffing. The Assessment Nurse conducts in-person clinical assessments for members to evaluate their health and functional needs and supports care planning by identifying unmet needs and collaborating with interdisciplinary teams to improve member outcomes. Base pay range

This range is provided by Amergis Healthcare Staffing. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more. Location : Boston (south of I-90 and Quincy) with in-home assessments for mostly geriatric patients. Schedule : Monday–Friday, 8:30 AM – 5:00 PM (no weekends, holidays or on-call) Caseload : 3 assessments per day (15/week) Compensation : Local: $60/hour Travel: $2,800/week Mileage Reimbursement: $0.70/mile Parking Reimbursed Position Overview

The Assessment Nurse conducts in-person clinical assessments for members to evaluate their health and functional needs. This role supports care planning by identifying unmet needs and collaborating with interdisciplinary teams to improve member outcomes. Key Responsibilities

Perform in-home assessments and clinical screenings using tools like the Minimum Data Set (MDS), PHQ-9, and others. Conduct medication reconciliation, assess adherence, and provide disease education. Escalate urgent concerns and refer members to appropriate services (e.g., behavioral health, LTSS). Document findings in electronic health records and care management platforms. Support telehealth assessments when appropriate. Collaborate with care teams to develop individualized care plans and determine program placement. EMR is ClinicalWorks. Required Qualifications

Active RN license in Massachusetts 1+ years of clinical experience (including clinical rotations) Experience with MDS (or Oasis) Strong clinical judgment, organizational, and communication skills Proficiency in Microsoft Office and willingness to use telehealth tools Preferred Qualifications

Bachelor’s Degree in Nursing Experience with electronic medical records Background in care management Bilingual in English and a second language For More Information
