Overview

We are seeking a patient-centered and detail-oriented

Clinical Pharmacist

specializing in obesity, diabetes, and weight management to join our team. In this role, you will collaborate with an interdisciplinary care team to optimize medication therapy, educate patients, and support safe, effective, and evidence-based treatment plans. This remote position leverages telehealth technology to deliver high-quality, patient-focused care while supporting the management of complex medication regimens for Type 2 Diabetes. Key Responsibilities

Patient Care & Medication Management Conduct virtual medication therapy management (MTM) sessions, including comprehensive medication reviews and reconciliation. Develop and monitor patient-specific pharmacotherapy plans for obesity, diabetes, Type 2 diabetes and related conditions. Provide patient counseling on proper medication use, dosage, administration, potential side effects, and adherence strategies. Monitor therapy outcomes, assess patient response, and identify opportunities for medication optimization. Proactively address medication-related problems and ensure therapies align with evidence-based guidelines.

Collaboration & Communication Partner with physicians and dietitians to deliver integrated, multidisciplinary care. Communicate recommendations to providers regarding therapy adjustments, potential drug interactions, and optimization strategies. Serve as a point of contact for medication-related questions from patients and providers

Clinical Documentation & Compliance Maintain thorough and timely documentation in the electronic health record (EHR) Ensure compliance with telehealth, pharmacy practice, and

state regulations. Stay current on evolving clinical guidelines, pharmacotherapy options, and new obesity/weight management treatments.

Required Qualifications

Doctor of Pharmacy (PharmD) degree from an accredited institution. Active and unrestricted

South Carolina pharmacist license

; Board Certification (BCPS, BCACP, or similar) preferred. (Other state licenses are a plus and/or willingness to add new ones as we expand into new markets in the future.) 3+ years of clinical pharmacy experience in weight management, endocrinology, or primary care. Experience with and strong knowledge of obesity/diabetes pharmacotherapies and protocols. Proficiency and comfort with telehealth technology and electronic medical records (EMR).

Excellent communication and patient education skills, with the ability to engage and motivate patients remotely. Strong clinical judgment and problem-solving skills for complex medication management. Proactive and solution-oriented mindset; comfortable identifying gaps and taking ownership of improvements. Entrepreneurial spirit with the ability to adapt quickly, wear multiple hats, and thrive in a fast-paced, early-stage startup environment. Detail-oriented with effective time management and organizational abilities. Comfortable working independently in a remote, technology-enabled environment.

Preferred Qualifications

Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS). Experience with motivational interviewing and behavioral support for medication adherence. Familiarity with remote patient monitoring and integrated care team workflows.

Work Environment

This is a fully remote position requiring high-speed internet, a private workspace, and comfort with virtual healthcare platforms. Interactions with patients occur via secure video calls, messaging, and phone consultations.
