Our client is seeking a skilled
Inpatient Clinical Pharmacist
to join their hospital pharmacy team. This role offers the opportunity to make a meaningful impact on patient outcomes through direct pharmaceutical care, clinical collaboration, and expert medication management in an inpatient setting. Key Responsibilities Provide direct pharmaceutical care to hospitalized patients, ensuring safe, effective, and evidence-based medication use. Review and verify physician medication orders for accuracy, dosage, and clinical appropriateness. Participate in daily medical rounds and collaborate with physicians, nurses, and other healthcare professionals to optimize therapy plans. Monitor patient progress and adjust medication regimens based on lab results and clinical outcomes. Prepare and dispense sterile and non-sterile compounded medications in accordance with regulatory and hospital standards. Identify, prevent, and resolve adverse drug reactions, therapeutic duplications, and drug interactions. Document clinical activities, interventions, and patient education in the medical record. Provide counseling to patients and families on medication use prior to discharge. Support formulary management, antimicrobial stewardship, and quality assurance initiatives. Contribute to ongoing process improvement and patient safety programs. Qualifications Doctor of Pharmacy (PharmD)
from an accredited school of pharmacy. Active pharmacist license
in good standing BLS certification
required;
ACLS certification
within 1 year of hire. Hospital or clinical pharmacy experience
preferred; PGY-1 residency is a plus. Must successfully pass a
background investigation
and meet requirements under the Indian Child Protection and Family Violence Prevention Act (ICPFVP). Valid, unrestricted, and insurable
driver's license . Bilingual skills
in English and Navajo preferred. Knowledge & Skills Comprehensive understanding of pharmacotherapy, aseptic technique, and inpatient medication systems. Ability to interpret clinical data, lab values, and vital signs to guide therapy. Strong collaboration and communication skills within a multidisciplinary team environment. Detail-oriented with strong critical thinking and problem-solving skills. Commitment to confidentiality, ethics, and excellence in patient care.
