Brown University Health . SUMMARY: Reports to the Pharmacist-in-Charge. Under general supervision of a Pharmacy Supervisor, is responsible for the interpretation, evaluation, preparation and dispensing of medications pursuant to a legitimate order in keeping with established policies, procedures, and standards of practice and scope of pharmacy services of the respective affiliate. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use. Oversees and closely reviews the work of pharmacy technicians and ancillary support personnel. Responsibilities

Know and act in accordance with the principles of the Brown University Health Corporate Compliance Program and Code of Conduct. Receive patient medication orders as written by a physician/practitioner; review for completeness and therapeutic appropriateness considering patient status, concomitant therapy, and developmental stage. Consult with prescribers to initiate interventions that optimize therapeutic efficacy, minimize risks, and improve cost effectiveness. Apply pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective drug use for patient populations served. Provide drug information and consultation to prescribers, nurses, and other health care professionals regarding drug therapy, interactions, side effects, allergies, discrepancies, stability problems, and administration. Proactively review and approve medication orders in LifeChart (drug, dose, form, frequency, duration); ensure patient-specific parameters are present (diagnosis, allergies, weight, age, etc.). Supervise pharmacy interns and technicians to ensure performance standards and regulatory compliance. Identify, document, resolve, monitor, and prevent drug therapy problems (contraindications, duplications, interactions); engage with other clinical pharmacists for complex situations. Integrate patient-focused medication therapy management and pharmaceutical care in accordance with clinical pathways or guidelines. Participate in pharmacokinetic dosing and monitoring; recommend dose adjustments and lab monitoring as required. Ensure compliance with target drug programs, formulary management, medication management initiatives, and adverse drug event monitoring programs. Document and report clinical interventions, medication errors, near misses, and adverse events per established protocols. Participate in emergency code blue response; attend multi-disciplinary rounds as time allows. Review and approve formulations for sterile products; perform in-process and final checks for medications prepared by technicians; ensure accuracy, sterility, and stability of compounded products. Participate in education and training of healthcare practitioners, including residents, students, interns, technicians, physicians, nurses, and other care professionals on matters related to medication use. Serve as preceptor to pharmacy residents and students. Participate in continuous quality improvement activities and total quality management initiatives, including adverse drug reaction and intervention and medication incident reporting programs. Coordinate transitions of care as appropriate. Qualifications

Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy. Current unrestricted licensure as a pharmacist in Rhode Island, or a 90-day temporary pharmacist license with full licensure obtained prior to expiration; ongoing unrestricted licensure in RI. Current American Heart Association Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) within 120 days of hire. Postgraduate residency (PGY-1) in pharmacotherapy preferred; PGY-2 in area of specialty strongly preferred if applicable. Clinical experience may be considered in lieu of formal training. Effective interpersonal and communication skills; strong organizational and presentation skills. Knowledge of technology in the pharmacy environment (pharmacy information systems, automated dispensing, software, equipment, automation); proficiency with MS Word, Excel, Outlook, and web browsers.
