We are looking for an individual who has a special interest in cancer care. An individual who is dynamic, organised, flexible, reliable & compassionate to support patients and their families/carers with an UGI cancer. This would involve patients with an oesophago‑gastric or hepatobiliary cancer from diagnosis, through treatment and at end of life.

The post holder will be an effective core member of our Upper G.I Multidisciplinary team (MDT) demonstrating the highest standard of advanced supportive care in the management of patients with an UGI cancer.

They will be responsible for working as an autonomous expert practitioner, making critical holistic clinical decisions through assessment, monitoring and planning. Facilitating timely investigations, coordinating care & providing information to enable patients to make informed choices.

A high proportion of our patients diagnosed with an UGI cancer have a palliative diagnosis; therefore the post holder will need to be prepared for the distressing nature of this role and have mechanisms in place to manage their psychological wellbeing.

Main duties of the job

Use specialist nursing skills to assess the physical and psycho‑social needs of patients within the defined caseload.

Perform independent assessments of patients within nurse‑led clinics & telephone clinics and manage outcomes appropriately.

Respond to helpline calls and act appropriately using specialist knowledge and skills.

Support patients within the community setting & avoid hospital admissions when appropriate.

Assist patients to contribute to the development of their own care plan, empowering them to make decisions at times of anxiety and uncertainty.

Participate in regular in‑patient ward reviews, assess patients and implement care as appropriate.

Safely request & interpret clinical investigations and respond as appropriate.

Communicate advanced information with patients, carers and health professionals, disseminating complex and potentially distressing information.

Perform procedures such as drain or suture removals, or be willing to learn.

Work well independently and be a supportive team member.

Attend and contribute to weekly MDT meetings, action outcomes and assist with recording cancer data.

Attend and support the GUTSY patient support group; participate in patient education and support needs.

Job responsibilities 1. Clinical Responsibilities/Key Responsibilities

Coordinate the patient pathway to ensure timely treatment and prevent breaches within Gloucestershire and liaise with tertiary centres.

Assist patients to develop their own care plan and empower them to make decisions.

Participate in regular in‑patient ward reviews, assess patients and implement care as appropriate.

Safely request & interpret clinical investigations and respond.

Communicate advanced information with patients, carers and health professionals.

Retrieve, analyse and interpret information from computer‑based systems, maintain clear and contemporary records.

Be willing to work between Gloucester and Cheltenham hospital sites.

Be flexible with working day and hours.

Possess excellent communication skills and be willing to undertake Advanced Communication skills training.

Possess or be willing to work towards Independent & Supplementary prescriber qualification.

Possess or be willing to work towards recognised qualification covering Physical Examination and Clinical Reasoning.

Possess or be willing to work towards non‑medical radiology request qualification.

Coordinate pathways to receive timely treatment and update when referred to or from tertiary centres.

Liaise and facilitate referrals to Queen Elizabeth hospital.

Be involved in initiatives to enhance patient experience, improve outcomes and service improvement.

Be involved in developing new nurse‑led services.

Support changes within practice to enhance service delivery.

Use advanced communication to disseminate complex, sensitive information that may be distressing.

Person Specification Qualifications

Registered nurse on relevant part of the NMC register.

Evidence of ongoing professional development and clinical experience in cancer or palliative care.

Evidence of Masters level learning or willingness to undertake.

Non‑medical prescribing.

Advanced Communications skills course.

Level 2 or above psychological skills training.

Knowledge

Acts as a role model and leads by example to reinforce Trust values and behaviours.

Demonstrates ability to keep up to date with professional issues that influence practice.

Contributes to development, implementation, monitoring and evaluation of policies/protocols/guidelines.

Experience

Substantial experience at Band 5 with specialist clinical skills in upper GI/oncology/palliative care.

Leadership and management experience.

£38,682 to £46,580 a year (pro rata if part‑time)
