Position Highlights

• Physician-led, patient-centered, team-based environment
• Provide care across the continuum, focusing on value-based care with a robust support team of specialists
• Procedures may include: simple skin surgery, fracture care, joint injections, maternity care, IUDs, etc.
• Full-time, schedule typically 4 or 4.5 workdays per week
• Must be comfortable working with advanced providers

Required Qualifications

• Unrestricted medical licensure in Indiana or ability to obtain prior to start
• Board certified or board eligible in Family Medicine, Internal Medicine, or Med-Peds
• Active and unrestricted DEA License or ability to obtain prior to start

Salary & Employment Details

The salary range for this role is $226,000 to $366,000 per year. This is total cash compensation at target. Actual range and pay mix vary based on experience, metrics, and other factors. Benefits include eligibility per company policy.
