Our Assertive Community Treatment (ACT) program brings together therapists, nurse practitioners, case managers, and peer specialists to deliver wraparound services in the field. We’re looking for a strategic, compassionate leader to guide this work. If you’re a licensed clinician with strong leadership skills and a passion for recovery-oriented care, you belong here. About the Role We’re hiring a Director of ACT Services to oversee our mobile, multidisciplinary ACT team serving adults with serious mental illness, often navigating co-occurring conditions, housing instability, or justice involvement. You’ll lead with both heart and rigor, ensuring clinical excellence, operational smoothness, and fidelity to the ACT model. You’ll collaborate closely with internal teams and external partners to reduce hospitalizations, promote community integration, and ensure that no one falls through the cracks. What You’ll Do Provide clinical oversight across a diverse team of case managers, therapists, nurses, and peer support specialists Lead daily team meetings to coordinate care, review risks, and assign tasks Maintain fidelity to ACT standards, including caseload ratios, service availability, and mobile outreach Review and approve treatment plans, ensuring cross-disciplinary integration Respond to psychiatric crises and coordinate safety planning and de-escalation Build and sustain community partnerships (e.g., hospitals, housing, courts, detox programs) Track and improve outcomes like hospital readmissions, client engagement, and housing stability Ensure compliance with Medicaid documentation and grant reporting Provide training in trauma-informed care, harm reduction, and motivational interviewing Champion a culture of equity, inclusion, and recovery What You Bring Master’s degree in Social Work, Counseling, Psychology, or related field 5–7 years of experience working with adults with serious mental illness 3+ years of supervisory experience in a behavioral health setting Knowledge ofthe ACT model, Medicaid billing, and integrated care best practices Strong leadership, crisis response, and interdisciplinary coordination skills Experience with EHRs (Athena preferred) and clinical assessments like IM+CANS Certified in evidence-based modalities (DBT, CBT, Harm Reduction, etc.) Experienced in forensic or street outreach work Bilingual and culturally responsive to Chicago’s diverse communities Why Join Vivia Health?
