Overview Provides medication-related support to the pharmacy team, population health department, and ambulatory clinics. Under the supervision of a pharmacist, the pharmacy technician engages with patients to assist in closing care gaps and ensuring optimal medication management.
This is a one-year grant-funded position
Responsibilities
Track and analyze data and reports from different sources such as medication adherence reports and patient charts
Perform outreach calls for medication adherence refill reminders and refill requests
Collect medication histories utilizing different sources such as electronic medical records and pharmacy technology platforms to assist pharmacists with medication reconciliations
Communicate with community pharmacies to assist patients and providers with medication refills, medication reconciliation, and troubleshooting access to medications
Assist with closing gaps in care as measured under value-based contracts including upside and downside arrangements
Maintain appropriate documentation related to medication outreach in patient electronic medical records
Develop relationships with providers and other clinical staff to facilitate effective communication and performance improvement
Provide support to pharmacists and other clinical personnel in the ambulatory clinics and community offices
Participate in routine meetings discussing opportunities for quality and process improvements
Qualifications
High school diploma or equivalent required
Certification as a Pharmacy Technician by the National Pharmacy Technician Certification Board or enrollment in an approved Pharmacy Technician program. Enrollment in a medically related training program (e.g., RN, LPN) is also acceptable
Experience in a community pharmacy setting required
Familiarity with HEDIS requirements
Knowledge of medical terminology and coding
Excellent customer service and interpersonal skills
Strong oral and written communication skills; proficient in English
Spanish or other second language preferred
Proficient with Microsoft Office Suite
Pay Range USD $48,237.00 - USD $60,000.00 /Yr.
