Job Purpose

Assists in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs in accordance with the patient's assessment and care plan, as directed by the Supervisor. Key Responsibilities

Conduct assessments and gather information by interviewing residents, family members, legal representatives, and significant others to develop individualized recreation programs that accommodate functional levels, needs, and interests. Update resident recreation programs as needed to accommodate individual needs and preferences and to protect and promote residents’ rights. Work cooperatively with the interdisciplinary team to develop, implement, and evaluate the plan of care. Attend care conferences and develop activity interventions as part of the comprehensive plan of care. Communicate resident concerns and responses to interventions to the interdisciplinary team members and direct care staff. Schedule recreational activities to accommodate residents’ hours (morning, afternoon, evening, weekend) that appeal to all age groups in a variety of locations and include special seasonal events. Post monthly activities calendar. Provide activities to residents confined to their rooms that reflect life‑long interests such as music, reading, visits with people of shared interests, and in‑room projects that the resident can work on independently or with staff assistance. Provide structured activities to meet the needs of cognitively impaired residents such as music events, gross motor games, or outdoor walks. Seek ways to adapt activities so that all residents can participate, such as segmenting tasks into simple steps, providing verbal prompts, physical support, demonstration, and hand‑over‑hand assistance when the resident is unable to complete a task independently. Develop and implement procedures to ensure that all residents are informed activities are occurring, that staff encourages participation, and that transportation is provided. Record progress notes in the clinical record including subjective findings, objective symptoms, observations of behavior, interventions provided, and residents’ responses to the interventions. Audit chart entries written by recreation services staff for completeness and accuracy. Perform quality assessment and assurance functions, including daily regulatory compliance rounds, observation of activities in progress on units, record reviews, and interviews with staff members, residents, families, and administration. Identify problems in recreation services staff performance and institute corrective action. Develop and update recreation services policies and procedures that reflect the philosophy and mission of the facility, professional standards of activities practice, and state, federal laws and regulations. Communicate with the nursing home administrator on a daily basis to discuss resident care, personnel, and budget issues. Develop staffing plans and recommend to the nursing home administrator numbers and types of personnel needed to meet resident needs in compliance with state and federal laws and regulations. Participate in the development of the department budget and provide relevant financial information to the nursing home administrator regarding department financial needs and status. Knowledge, Skills, Abilities

Participate in center/agency surveys (Licensure/JCAHO) and any subsequently required reports. Attend and participate in continuing educational programs to keep abreast of changes in your field and to maintain current license/certification as required. Attend and participate in mandatory in‑services. Honor patients/residents’ rights to fair and equitable treatment, self‑determination, individuality, privacy, property, and civil rights, including the right to wage complaints. Comply with corporate compliance program. Report job‑related functions/tasks that involve occupational hazards, including exposure to blood and body fluids and others as necessary. Follow established safety regulations, including fire protection and prevention, smoking regulations, infection control, etc. Follow established safety procedures when performing tasks and/or working with equipment. Minimum Education Required

Requires minimum two (2) years college education with courses in recreation, education, social work, other behavioral sciences or clinical services. Minimum Experience Required

Two (2) years’ experience in a social or recreational program within the past five years, with one year full‑time employment in a patient/resident activities program in a health care setting. Family Makes Us Stronger
