Heart Failure Nurse Practitioner, Chobanian & Avedisian School of Medicine, Cardiovascular Medicine
Boston Medical Center (Boston University Medical Campus) seeks two full-time advanced practice providers to join a multidisciplinary cardiology team. The roles will focus on heart failure outpatient clinic management and intravenous/subcutaneous infusion therapy.

Responsibilities HF NP Responsibilities
Provide comprehensive management of heart failure patients in the outpatient clinic.
Coordinate patient education and perform intensive phone triage for urgent heart failure concerns.
Offer guidance on symptom management, medication adjustments, and urgent care needs to reduce hospital readmissions.
IV NP Responsibilities
Clinical management of cardiology patients receiving intravenous and subcutaneous therapies in an outpatient clinic, infusion suite, or hospital setting.
Initiate and manage IV infusion treatments, monitor patient responses, and coordinate ongoing cardiovascular care.
Triage cardiology-related infusion requests from the Emergency Department, outpatient clinics, and inpatient teams to determine appropriate outpatient follow-up or HF infusion clinic care.
Support patients receiving advanced treatments such as subcutaneous therapies for amyloid cardiomyopathy and other cardiology infusion therapies.
Qualifications
Strong clinical judgment and ability to manage patients autonomously within defined protocols.
Experience in cardiology and heart failure clinical practice is preferred.
Licensed to practice in the state of Massachusetts.
Ability to order and interpret diagnostic tests (labs, ECGs, echo) to guide treatment decisions.
Participate in care coordination including scheduling, post-treatment follow-up, and collaboration with cardiologists, NPs, and cardiology pharmacists.
