The Case Manager is a clinician who coordinates quality health care services across the continuum of care. This role requires knowledge of development stages for infants, children, adolescents, pediatrics, adult and geriatric populations and the cultural diversity that influences responses to illness. Required Qualifications
Working knowledge of Intequal IS-SI Criteria Working knowledge of Medicare, Medi-Cal, NCQA, Heidis and other criteria identified by the Quality/Care Management Department Minimum of 1 year discharge planning experience Working knowledge of managed care and capitation operations Current California Registered Nurse license All external candidates must have a Bachelor of Science in Nursing Must possess the skill, knowledge and ability essential to the successful performance of assigned duties Must demonstrate customer service skills appropriate to the job Excellent written and verbal communication skills in English Ability to maintain a work pace appropriate to the workload Ability to establish and maintain effective working relationships across the Health System Preferred Qualifications
Certified Case Manager Certification Knowledge of University and medical center organizations, policies, procedures and forms Total Compensation
