The post holder will work in bothpatient-facing and non-patient-facing roles. The post holder may be askedto take responsibility for areas of chronic disease management, includingrespiratory conditions, and should demonstrate both experience in primary careand interest in these clinical areas. The post holder may provideprimary support to general practice staff with regards to prescription andmedication queries. They will help support the repeat prescription anddispensing system, help with acute prescription requests, support medicinesreconciliation on transfer of care and systems for safer prescribing. The post holder will provideclinical support on medicines optimisation and quality improvement and willsupport and manage some aspects of the quality and outcomes framework,dispensing services quality scheme and enhanced services. The post holder will befamiliar with the QOF domains, ensuring that all clinical reviews undertakenare correctly targeted and coded to help the surgery continue their outstandingQOF achievement and be familiar with the Somerset prescribing formulary andscorecard indicators, aiming to optimise prescriptions to align with these. The post holder will be either anindependent prescriber or be committed to obtaining this qualification. Main duties of the job

Review the on-going need for each medicine. Run own long-term condition clinics where responsible forprescribing as an independent prescriber Undertake clinical medication reviews with patients andproduce recommendations for/implement changes in prescribing and monitoring. Answers relevant medicine-related enquiries from GPs, otherpractice staff, other healthcare teams and patientswith queries about medicines. Suggesting and recommending solutions. The post holder will ensure thatthe practice integrates with community and hospital pharmacy to help utiliseskill mix, improve patient outcomes, ensure better access to healthcare andhelp manage workload. Qualifications

Masters degree in pharmacy (MPharm or equivalent) Independent prescriber or working towards/intent of gaining independent prescribing qualification Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience Independent prescriber or working towards/intent of gaining independent prescribing qualification Independent prescriber or working towards/intent of gaining independent prescribing qualification Interest in working in a primary care setting Interest in training, education or mentorship Experience

Minimum of 3 years post-qualification experience. In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare. An appreciation of the nature of general practice An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing Excellent interpersonal, influencing and negotiating skills Excellent written and verbal communication skills Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (i.e. patients) Is able to plan, manage, monitor, advise and review general medicine optimisation Able to obtain and analyse complex technical information Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer when appropriate Able to work under pressure and to meet deadlines Produce timely and informative reports Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers Work effectively independently and as a team member Demonstrates accountability for delivering professional expertise and direct service provision Disclosure and Barring Service Check
