Position Summary This position provides education to patients and families about advanced care planning, acts as a patient advocate or liaison to ensure appropriate utilization of advanced care planning services, consults with patients and families upon referral, and delivers pastoral care to patients, families, students, and staff as a member of the Pastoral Care team.
Duties & Responsibilities
Provide education to patients and families about advance care planning.
Act as patient advocate or liaison between patient, family, and healthcare providers to ensure appropriate utilization of advanced care planning services.
Consult with patients and families upon referral for advance care planning.
Provide patient consults and follow‑up regarding Advance Directives upon referral, and offer additional referrals or resources as appropriate.
Document consultation outcomes and retain records per Pastoral Care process.
Plan, implement, and evaluate the advance care planning program activities to educate community agencies, physicians, other health care personnel, and the general public about available services.
Develop community engagement strategies, oversee volunteers, and implement advance care planning program development.
Provide pastoral ministry in a timely manner, respecting cultural and ethnic diversity and social conditions without imposing personal perspectives.
Assess patient and family spiritual needs and assist families in using faith to cope with illness, trauma, and stress; visit newly admitted patients, critical care areas, and follow‑up as appropriate.
Provide crisis intervention and pastoral counseling regarding religious/spiritual and emotional consequences of illness and/or disability, mobilizing family and community spiritual resources.
Offer crisis ministry and bereavement/grief ministry to patients, families, and clinical personnel.
Use empathic reflection, listening skills, and appropriate religious/spiritual resources.
Maintain working relationships between Pastoral Care, medical staff, nursing staff, and other departments.
Contribute to the department’s continuous quality improvement plan.
Support an interdisciplinary approach to pastoral care and advance directives consultation.
Participate in appropriate committees.
Qualifications Minimum Qualifications
Master’s degree from an accredited theological school or religious studies, ordination or commissioning for ministry by a recognized religious group, and a current endorsement for chaplaincy.
Minimum of one year clinical chaplaincy experience with knowledge/background in Advance Directives.
Additional training, education, or experience in Advance Directives preferred.
Formal ethics training and education preferred.
Eligible for certification as Board Certified Chaplain; completion of board certification preferred (must have 4 units of CPE and certification obtained within 18 months of hire).
Self‑awareness as a pastoral caregiver, including attitudes, values, and the impact of ministry on people.
Knowledge of the hospital/clinic system and multi‑disciplinary teams.
Empathy for the problems experienced by patients and families; commitment to help the distressed; interpersonal skills for working with distressed patients, families, and staff.
Demonstrated computer literacy and ability to document and retrieve information through computer applications.
Ability to work independently and exercise good judgment.
Excellent interpersonal skills to establish and maintain professional relationships with members of the health care team.
Must be able to work Tues – Friday (09:30 – 18:00) and Saturday (07:00 – 15:30) or Sunday (07:00 – 15:30) and Mon – Thurs (09:30 – 18:00).
