Position Summary This position provides education to patients/families about advanced care planning. The candidate acts as patient advocate or liaison between patient, family, and healthcare personnel to ensure appropriate patient utilization of advanced care planning services. The role consults with patients/families upon referral for advanced care planning and is responsible for the delivery of pastoral care to patients, their families, students, and staff as a member of the Pastoral Care team.

Act as a patient advocate or liaison to ensure appropriate utilization of advanced care planning services.
Consult with patients and families upon referral for advanced care planning.
Deliver pastoral care to patients, families, students, and staff as part of the Pastoral Care team.
